Effective April 1, 2021, we are operating under a new name, Home and Community Care Support Services, with a mandate focused on providing home and community care and long-term care home placement. The health system planning and funding functions previously handled by the Local Health Integration Networks have transferred to Ontario Health, the agency responsible for ensuring that Ontarians receive high-quality health care services, where and when they need them.
Although the name has changed, people living in the community can continue to count on Home and Community Care Support Services to deliver the high-quality, patient-centred home and community care services they need.
